Registration form

The club has one form, and it is here so you can read it — or fill it in — at home rather than at a table in a busy library.

Nothing has to be done before you come

There is no booking, no waiting list, and nothing to pay. Bring your child along on a Saturday morning and say hello. Graham hands this form out at the club and it can be filled in there, so downloading it first is entirely optional. If you would rather ask something before you arrive, the contact page has the details, and find us has the directions.

What the form is for

It is how the club records whether a photograph of your child may appear on this website, and whether your child may be named alongside it. Those are two separate questions and you can answer them independently — the safeguarding policy explains how the club treats the answers, including what happens when a question is left blank.

It also gives Graham a way to reach you: a phone number, so he can contact you quickly if something happens at a session, and an email address, so he can let you know when something new goes on the website. The form sets out what it collects, what each part is for, and how long it is kept. The privacy notice does the same for the information that appears on this website.

Download and print it

Download the registration form as a PDF — one side of A4, 212 KB — print it, fill it in, and bring it along to a session. The club meets on Saturdays, 10am–12 noon, upstairs at Ferndown Library. If you would rather not print anything, just ask Graham for a paper copy when you arrive.

Where the filled-in form goes

Back to Graham, on paper. There is nothing to submit on this website — this club deliberately runs no online forms, and a completed sheet carries your child’s date of birth and your signature, which are better handed over than emailed.

Graham keeps the forms himself. They are not held on this website and not in any system this website uses.
